Defects or Issues Identified

"Bay 01 - FWD, outboard lashing platform on both the port and starboard sides does not have adequate fall protection. Portable safety railings need to be provided for. All outboard lashing platform safety railing has a gap that would allow a person to fall through. The extra protection which was once there has been removed because of damage or corrosion. At Bay 57 port and starboard the access up to the lashing platforms is restricted because of the safety railing design. A fall from height risk exists here also due to a lack of railing height."

  • "I have been informed that this vessel will no longer be returning to Australia by the Chief Officer. Myself the PIC and the head Lasher completed a risk assessment and all were happy to use the harness in the areas of concern by 'Trained' personel only."
